Abstract

This article presents an analysis on the needs for at-home nursing care and care through the Unified Health System, after the discharge, of adults and dependent elderly. Research undertaken in two phases, between October 2012 and July 2013. The first, exploratory research involved 217 patients hospitalized to assess the capacity for self-care and care demands after discharge according to Orem’s theory. The second phase, based on the Grounded Theory, was developed at six homes of people with self-care deficit through semistructured interview. It was evidenced that the patients and caregivers evidenced difficulties in attending to their needs, highlighting the burden, lack of professional support and difficulties to respond to the demand for material and equipment. In conclusion, the effective implementation of Home Care is fundamental for care integrality.
